First and second prizes will be determined by the closet guess WITHOUT GOING OVER the amount of Botox bottles in OU Beauty’s Botox bottle (as picture to the right). Other prizes will include treatments & products from manufacturers.
No one knows the amount in the bottle until the count is made. The count will be held Friday, May 15th, 2015. Guesses must be made, in person, on OU Beauty’s Contest Form, at OU Beauty located at 130 N Brand Blvd., Glendale CA 91203.
Contest Rules:
To guess how many empty Botox bottles are in the our Botox Bottle Jar, all guesses must be made in person at OU Beauty Medical Spa located at 130 N Brand Blvd, Glendale CA 91203. Guesses must be made on our Entry Form and will require contact phone number and signature for prize verification. ID may be asked for to match up with the entry form.
The guess closest to the actual amount, without going over, will win First Prize. The second closest guess will win 2nd Prize. In case of a tie for first, there will be a blind drawing (also on May 15th, 2015, after the count is finalized) of the guesses that are tied. If there are more than 2 tied for 1st (or 2nd), then there will be multiple drawings to determine the winner (‘Round Robin” format).
Employees, owners, their family members and business associates are not eligible for prizes in this contest. Contestants may be vetted for these qualifications before prizes are remitted. If a winner is disqualified for their association with company or it’s people, the prize will go to the next ‘best guess’ that is a qualified contestant. No purchase is required. Just come in and give us your best guess (without going over).
To adhere to the many rules and Federal regulations of drawings for prizes and gifts, one ‘Guess Form’ may be remitted per person only. If multiples Guess Forms are identified, all ‘Guess Forms’ of that person will be left out of contest. Thanks for your cooperation.
The rest of the prizes (i.e., after 1st and 2nd prizes are determined) will be determined through a blind drawing held Saturday, May 16th, 2015.
